Patient-centered outcomes and trials of hydroxyethyl starch

چکیده
Meybohm and colleagues [1] propose that hydroxyethyl starch (HES) may be used safely in hypovolemic patients by applying a clinical algorithm and by restricting the dose administered. The authors question the validity of the results of the two trials that constitute over 60% of current data [2,3] and misleadingly state that in the Crystalloid vs. Hydroxyethyl Starch Trial (CHEST), HES administration did not increase the use of renal replacement therapy by referring to the adjusted analyses that were published in the electronic supplement [2]. The unadjusted analysis was pre-specified as the principal outcome measure and is the appropriate measure to influence clinical practice. The authors also ignore the consistent signal of harm associated with HES, specifically increased mortality and use of renal replacement therapy that is evident despite wide variations in aggregate doses of HES in the three major clinical trials: 70 ml/kg in the Efficacy of Volume Substitution and Insulin Therapy in Severe Sepsis trial [4], 44 ml/kg in the Scandinavian Starch for Severe Sepsis/Septic Shock study [3], and 5 ml/kg in CHEST. Meybohm and colleagues make no comment that adverse effects of HES represent an overall toxic effect caused by increased tissue accumulation that is recognised as a dose-dependent, generic HES effect [5]. The ‘presumably correct indication’ and the algorithm they propose have not been validated nor are they supported by any credible clinical evidence. Their proposed algorithm and target population must be evaluated in rigorously conducted randomized controlled trials before being considered for adoption into clinical practice. Given the consistent evidence that HES is nephrotoxic and may increase mortality [6], it is doubtful that institutional ethics committees would approve such a trial, or that informed patients would consent to participate.

Comparison of 6 % hydroxyethyl starch and 5 % albumin for volume replacement therapy in patients undergoing cystectomy (CHART): study protocol for a randomized controlled trial
BACKGROUND The use of artificial colloids is currently controversial, especially in Central Europe Several studies demonstrated a worse outcome in intensive care unit patients with the use of hydroxyethyl starch. This recently even led to a drug warning about use of hydroxyethyl starch products in patients admitted to the intensive care unit. The data on hydroxyethyl starch in non-critically il...

OBJECTIVE To assess the effects of fluid therapy with hydroxyethyl starch 130/0.38-0.45 versus crystalloid or albumin on mortality, kidney injury, bleeding, and serious adverse events in patients with sepsis. DESIGN Systematic review with meta-analyses and trial sequential analyses of randomised clinical trials. DATA SOURCES Cochrane Library, Medline, Embase, Biosis Previews, Science Citati...
متن کاملAssociation of hydroxyethyl starch administration with mortality and acute kidney injury in critically ill patients requiring volume resuscitation: a systematic review and meta-analysis.
IMPORTANCE Hydroxyethyl starch is commonly used for volume resuscitation yet has been associated with serious adverse events, including acute kidney injury and death. Clinical trials of hydroxyethyl starch are conflicting. Moreover, multiple trials from one investigator have been retracted because of scientific misconduct. OBJECTIVES To evaluate the association of hydroxyethyl starch use with...
متن کاملEffect of hydroxyethyl starch on acidosis in patients with aluminum phosphide poisoning
Background: Given the fact that various studies have reported the positive effects of hydroxyethyl starch therapy in controlling shock, this study aimed to compare the effects of hydroxyethyl starch on modifying acidosis and treating patients with aluminum phosphide poisoning. Methods: This was a randomized clinical trial that was conducted on 60 patients with aluminum phosphide poisoning. We ...
متن کاملSystematic review of randomized clinical trials on the use of hydroxyethyl starch for fluid management in sepsis
BACKGROUND Patients with sepsis typically require large resuscitation volumes, but the optimal type of fluid remains unclear. The aim of this systematic review was to evaluate current evidence on the effectiveness and safety of hydroxyethyl starch for fluid management in sepsis. METHODS Computer searches of MEDLINE, EMBASE and the Cochrane Library were performed using search terms that includ...
